Often within 24–72 hours of your call, depending on the package. Tell the ward you are arranging home care and connect us with the discharge coordinator, we will handle the rest.
No. Recovery support is designed to end. Care does not have to be a long-term arrangement, it can be short-term support during recovery following a medical emergency, such as a stroke.
We can dramatically reduce the risks that cause readmission: missed medication, poor nutrition, falls and undetected decline. Our care staff spot problems early and escalate quickly.
